Dermatology drug manufacturing is dominated by topical formulations -- creams, ointments, gels, lotions, foams, and solutions. These semi-solid dosage forms require specialized mixing equipment (high-shear mixers, homogenizers), precise temperature control during formulation, and validated filling lines for tubes, jars, pumps, and airless dispensers. The manufacturing expertise is distinct from solid dose or injectable production.
Skin penetration is the formulation challenge. The stratum corneum (outermost skin layer) is an effective barrier that most drugs cannot cross easily. CDMOs need formulation scientists experienced with penetration enhancers, nano-emulsions, liposomal delivery, and permeation testing (Franz diffusion cells). The choice of vehicle (cream vs ointment vs gel) significantly affects drug delivery and patient acceptance.
Biologics are entering dermatology. Monoclonal antibodies for psoriasis (anti-IL-17, anti-IL-23), atopic dermatitis (anti-IL-4R, anti-IL-13), and other inflammatory skin conditions have transformed treatment. While these are manufactured as standard injectable biologics, the combination with topical products in treatment protocols means some CDMOs serve both modalities for dermatology sponsors.
